The Opportunity

Moonee Valley City Council is seeking a motivated and enthusiastic individual to provide care coordination and administrative support to the Community Support Service. The Community Support Team is responsible for facilitating residents' access to food security and transport, via the provision delivered meals, community dining and transport services that support residents' access to affordable and nutritious food. The Community Support team also assists residents to remain connected with social, wellbeing and medical support services through the provision of Community transport services.

Position Objective

  • To provide general and administrative support to the Community Support team to co-ordinate the delivery of consumer directed care.
  • To provide a high level of customer service and liaise with staff at all levels whilst maintaining a high level of professionalism, integrity and confidentiality.
  • Provide quality administrative support that facilitates the delivery high quality services which align with Council's funding obligations and compliance requirements.
  • Adopt process and procedures that advance operational efficiencies within the Community Support Team.
  • Support the Community Support Team to deliver services in accordance with Council Occupational Health and Safety Policies and Safe Work Procedures.

Key Responsibilities and duties:

  • Participate in the Community Support Phone Queue
  • Provide a high level of internal and external customer support in line with the organisation's customer experience procedures and protocols.
  • Assist with the onboarding and commencement of client's services and liaise with relevant stakeholders.
  • Complete rostering functions to support operational delivery of meals and transport services.
  • Escalate unresolved client issues and complaints (i.e. service dissatisfaction and change in care needs) to client's allocated contact.
  • Co-ordinate any changes required to clients' regularly scheduled services.
  • Respond to client non-responses, serious incidents, and emergencies in line with Departmental procedures.
  • Raise purchase orders and the receipt of goods and services in the financial system
  • Maintain accurate records, including the input of case notes and ensure client data is up to date within the Aged and Disability Services' Client Management System (CMS) and the My Aged Care portal.

Skills and Experience

  1. Tertiary qualifications in business administration or related field with little or no experience, or lesser relevant qualifications with some work experience.
  2. Demonstrated competence and experience in Microsoft Office software and Client Management Systems.
  3. Excellent verbal communication skills, particularly in relation to use of the telephone.
  4. An understanding of and capacity to communicate clearly with older people, people with disabilities and their carers.
