Adelaide Primary Health Network (Adelaide PHN) is an independent, not for profit, primary health care organisation based in Adelaide.

The Practice Facilitator will work as part of a dynamic team assisting general practices and primary care stakeholders across the Adelaide PHN region. A key component of the role is the ability to engage and build strong relationships with local GPs, practice nurses and practice staff, including but not limited to, provide hands on coaching, support and advice to:

This position is being offered in a full time or part time capacity and is funded until June 2026. Future employment is subject to ongoing funding.
